Horses have a lot of calcium in their diet. If a horse gets a kidney or bladder infection the debris can act as a focus for the calcium salts to precipitate out and, over a long period of time, cause a bladder stone to form. These are quite rough and jagged and cause trauma to the bladder which then leads to bleeding from the bladder wall, and blood in the horse's urine.
